REPORTING RELATIONSHIP

Reports to the Home Care Supervisor

SUMMARY

The Client Care Coordinator (CCC) processes incoming requests for service and coordinates and schedules Supportive Aides and Certified Home Health Aide assignments under the direction of the Home Care Supervisor and in collaboration with the Director of Nursing. The Client Care Coordinator is responsible for ensuring that all time sheets reflecting aide activity are received in a timely manner, and that all subcontractors comply with time sheet submission requirements pursuant to Contract. Performs all duties in accordance with the FCS values and Code of Conduct.

RESPONSIBILITIES

  • Accepts intra-agency referrals and hands off to Home Care Supervisor.
  • Refers all prospective private pay clients to Home Care Supervisor for intake. In absence of Home Care Supervisor, CCC will refer prospective private pay client to Director of Nursing.
  • Lists new referrals on white board in Home Care Office, reflecting date, payor source, type of care and hours.
  • Guides/assists clients to proper public funding programs when indicated to enable clients to have needed services without financial burden.
  • Maintains a schedule of home health aides and supportive aides and updates assigned Nurse Supervisor and Home Care Supervisor immediately of any schedule changes.
  • Updates Emergency Priority Codes in electronic chart as directed by Nurse Supervisors.
  • Follows up with subcontractor agencies every Wednesday to inquire of any schedule changes and immediately updates assigned Nurse Supervisor and Director of Nursing of any changes.
  • Documents aide call-outs in the electronic system and provides report to Home Care Supervisor weekly, no later than Wednesday of each month.
  • Tracks aides who consistently refuse cases and provides report to Director Nursing monthly, no later than the third Tuesday of every month.
  • Takes an active role in assisting aides to complete in-services on the electronic system; tracks compliance by accessing data from the electronic system, ensuring aides are paid for each in-service completed.
  • Obtains Certification of Need from client’s Health Care Provider, when necessary, pursuant to payor source requirements.
  • Updates client census weekly on white board in Home Care Office.
  • Keeps list of out-sourced clients, hours, subcontractor agency and payor source current on white board in Home Care Office.
  • Assists with payroll on bi-weekly basis, ensuring that all time sheets are received in a timely manner.
  • Follows up with subcontractors to ensure time sheets are received in a timely manner pursuant to the terms of the Contract.
  • Assumes other duties as assigned by the CEO, Home Care Supervisor and Director of Nursing.

WORKING RELATIONSHIPS
Works closely with the Director of Nursing, CEO, CFO, HR Manager, Program Directors and agency Staff. Meets with Board of Directors as needed. Maintains professional relationships in the community and with outside agencies pursuant to the FCS Code of Conduct.

WORKING ENVIRONMENT
This job operates in a professional office and field environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

PHYSICAL DEMANDS

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. The majority of work is light duty which requires standing, bending, speaking, and manual dexterity and near visual acuity. Seeing, smelling, repetitive motion and lifting objects weighing up to 50 pounds at a time are required for this position.

BACKGROUND/EDUCATION/TRAINING/EXPERIENCE

  • At least two years of proven home care management/administration experience
  • Bachelor’s degree preferred
  • Excellent communication or organizational skills; ability to focus on details
  • Proven interpersonal and relationship building skills
  • Knowledge of appropriate governmental regulations and
  • Must be computer
